Polyphonic Care By Dr Phil Lockie

Welcome! To support you during your care,  you have been given the opportunity to use our new metbolic health application, Polyphonic Care. Here at Dr Phil Lockie's Practice, we are bringing all of our specialist staff and facilities together to give you the very best care and support before, during and after your surgery or treatment.

Polyphonic Care can be downloaded by following  these 2 easy steps (You'll need to register first here or follow the QR Code below.)

1. Click on the appropriate link below or search for Polyphonic Care in either the Google Play Store or App Store® for iPhone and download to your device.
2. Once installed, open the App.  You then need to enter the hospital specific code below.
